Identity theft is the fastest growing financial crime in America - striking thousands of victims each year. In a matter of seconds, your personal information such as a social security number, a credit card number and/or an address can be stolen and used to obtain a new mortgage, a line of credit or additional credit cards.

You may never know that your identity has been stolen until unauthorized transactions appear on credit card statements, collection notices appear in the mail or loan applications are declined. In a short period of time, extensive damage can be done as a result of a stolen identity event.

Restoring one’s name and good credit is a time-consuming and expensive process. On average, it takes six months to detect identity theft and approximately 200 hours to recover from the crime. Victims must endure a number of costly steps to dispute fraudulent debts and accounts opened by an identity thief.

Zurvita Protection family plan covers the member, their spouse, domestic partner or significant other, any children between the ages of 18 and 25, and any dependents living with, and dependent upon, the member such as a parent or grandparent.

Restoration Services

Upon notification of an identity theft incident, paralegals will:

• Consult with the covered member to ascertain the severity of the theft and start the notification process.
• Prepare and send the customized, pre-completed, state specific “ID Recovery Kit™” to covered member via Federal Express overnight.
• Send all necessary electronic notifications.
• Contact all three credit reporting agencies to: obtain a free credit report for the covered member, place fraud alerts on the covered member’s credit records, and obtain a list of creditors involved with the fraud.
• Submit special Power of Attorney and ID Theft Affidavit to involved creditors requesting cancellation of member’s card and issuance of a new card.
• Notify the appropriate bank or agency if other forms of identification (such as an ATM card, driver’s license, social security card, or passport) were stolen or are missing, so that appropriate action can be taken and new forms of identification issued.
• Notify local authorities of ID Theft incident and help member obtain necessary reports.
• Follow up with creditors, where needed, to ensure that the matter has been properly handled.
• Provide fraud resolution, legal and emotional assistance from beginning to end.

Identity Theft Insurance

Each member will also receive up to $25,000 worth of insurance coverage* underwritten by a nationally recognized Insurance Carrier with an “A” or better AM Best Rating. This coverage will help members offset some of the costs of restoring their identity to its original status, including:

• Lost wages: $500.00 per week, for 4 weeks maximum
• Re-filing of loans
• Defense cost for certain civil law suits: Reasonable and necessary fees and expenses incurred in the United States by the insured with insurer’s consent for an attorney appointed by them for:
  • Defending any civil suit brought against the insured by a creditor, collection agency or other entity acting on behalf of a creditor for non-payment of goods or services or default on a loan solely as a result of a stolen identity event.
  • Removing any civil judgment wrongfully entered against the insured solely as a result of a stolen identity event.
• Reimbursement of fees: Reasonable and necessary costs incurred in the United States by the insured for:
  • Re-filing applications for loans, grants or other credit instruments that are rejected solely as a result of a stolen identity event.
  • Notarizing affidavits or other similar documents, long distance telephone calls and postage solely as a result of the insured’s efforts to report a stolen identity event and/or amend or rectify records as to the insured’s true name or identity as a result of a stolen identity event.
  • Up to six credit reports from established credit bureaus (with no more than two reports from any one credit bureau) dated within 12 months after the insured’s discovery of a stolen identity event.

Lost Credit Card Assistance Services

All Lost Credit Card Assistance (LCCA) members will be provided with toll free access to certified Identity Theft Recovery Specialists. These specialists will assist the members in the event a member misplaces or has their credit cards stolen.

Should this occur our Specialists will:

• Consult with the member to determine the severity of the event and begin the notification process. This includes obtaining credit reports from each of the three major credit reporting bureaus.
• Contact each credit card company while member is on the phone.
• Cancel the member’s affected credit cards.
• Request new replacement cards.
• Place fraud alerts with all three major credit bureaus.
• Assist member with interpreting their credit reports.
• Provide daily credit monitoring from all three bureaus for six months to help prevent any further fraud.

Registration of the member’s credit cards is not required. This safety measure prevents further fraud and/or identity theft.

All of our Call Center Personnel (ID Recovery Specialists) are certified paralegals with extensive legal and fraud experience. We train our Call Center Personnel in crisis intervention and provide them with continuing fraud education. Further, our paralegals work under the direction of our corporate attorney.

Unlimited Legal Care at Discounted Rates

With this program, identity theft victims can defend themselves against crimes committed by an identity thief. Beyond the $25,000 worth of identity theft coverage, our network of Plan attorneys have contracted to never charge more than $125.00 per hour, or when appropriate, give members a 40% discount off their usual and customary hourly rate, for all extended legal care.

Members may use our program to prosecute identity theft thieves. This is the only identity theft program in the country that provides unlimited discounted legal care, enabling members to both defend themselves against a theft, and prosecute thieves.

Free & Discounted Legal Care

Zurvita Protection members and their families will have access to a nationwide network of over 20,000 plan attorneys. These attorneys have contracted with Legal Club to provide our members with free and discounted legal services.

Upon enrollment, you will be referred to a plan attorney that speaks your language, practices the area of law required by your specific needs and is conveniently located.

Capped Hourly Rate

Legal Club plan attorneys contract to never charge more than $125.00 per hour, or when appropriate, give members a 40% discount off their usual and customary hourly rate, for legal care that goes beyond the free and discounted services. This is where tremendous savings are realized. After the free letters, calls and consultations take place, members and their attorneys will decide what course of action to take next. In most cases, members will have no choice and litigation may ensue. This is the point in time that Legal Club plan attorneys charge the reduced hourly rate for legal care. The savings with Legal Club will be thousands of dollars.

Retainers

In the case of extended legal care, plan attorneys may ask you for a retainer. Any retainer sought will be computed by multiplying the number of hours a plan attorney believes a case will take, by the appropriate discounted hourly plan rate. For instance; 10 hours x $125.00 = a retainer of $1,250.00. Any unused portion of the retainer will be returned to you.

Contingency Fee Discounts

The contingency fee discount will be a 10% reduction of the state maximum rate or the attorney’s usual rate, whichever is lower.

Law Areas Included

Legal Club does not exclude any type of law area. Many legal expense insurance products have waiting periods, pre-existing condition exclusions and limitations on usage. This is due to the fact that they have financial exposure to usage, just the same as an HMO. As such, many types of law areas are excluded in their plans. Legal Club has no exposure and, therefore, takes the opposite approach and encourages usage. In fact, 80% of all new members use the legal plan within the first 90 days after they join.
